Blood pressure management in acute stroke

چکیده
Correspondence to Professor Philip M Bath; [email protected] ABSTRACT Blood pressure (BP) is elevated in 75% or more of patients with acute stroke and is associated with poor outcomes. Whether to modulate BP in acute stroke has long been debated. With the loss of normal cerebral autoregulation, theoretical concerns are twofold: high BP can lead to cerebral oedema, haematoma expansion or haemorrhagic transformation; and low BP can lead to increased cerebral infarction or perihaematomal ischaemia. Published evidence from multiple large, high-quality, randomised trials is increasing our understanding of this challenging area, such that BP lowering is recommended in acute intracerebral haemorrhage and is safe in ischaemic stroke. Here we review the evidence for BP modulation in acute stroke, discuss the issues raised and look to on-going and future research to identify patient subgroups who are most likely to benefit.

Blood pressure management in acute stroke: does the Scandinavian Candesartan Acute Stroke Trial (SCAST) resolve all of the unanswered questions?
Whether raised blood pressure in patients with acute stroke should be treated is one of the major unresolved issues in acute stroke management, due mainly to a paucity of reliable data from sufficiently powered randomized controlled trials. However, the Scandinavian Candesartan Acute Stroke Trial (SCAST), recently published in The Lancet, adds important new information about the risks and benef...

Background: C.V.A is an important health problem. Efforts to reduce the incidence of C.V.A are essential and such attempts should be based on the knowledge of risk factors. In this study C.V.A risk factors were evaluated. Materials and Methods: We used a case control method. The cases comprised of 146 patients admitted to stroke unit with acute stroke. The controls comprised of 147 particip...
متن کاملBlood pressure in acute stroke.
Elevated systolic blood pressure levels (> or =160 mmHg) are a common complication of acute stroke, affecting up to 60% of patients, and providing an immediate management dilemma in the 40% of acute stroke patients on pre-existing antihypertensive therapy. There are theoretical reasons for both lowering blood pressure in the acute stroke situation, as well as leaving raised levels untreated. Fu...
متن کاملAcute treatment of blood pressure after ischemic stroke and intracerebral hemorrhage.
Blood pressure elevation in the setting of acute ischemic stroke and intracerebral hemorrhage is common. Controversy exists as to the optimal management of elevated blood pressure in this patient population. This article reviews the available clinical data that guides acute blood pressure management and how this may affect clinical outcomes. Furthermore, it examines how these data have led to c...
متن کاملBLOOD PRESSURE COMPONENTS AS PREDICTORS OF STROKE MORTALITY IN WEST SCOTLAND
The relative importance of systolic (SBP) versus diastolic blood pressure (DBP) and other combinations of SBP and DBP in the prediction of stroke have been re-examined in a long term cohort study of 10,541 men and women aged 45-64 in West Scotland. During a mean follow-up of 11.6 years 1, 616 deaths occurred, among which 160 (9.9% 80 male, 80 female) were due to stroke. In a multiple logi...
